1. If A happens, it's over (nothing will be of use)
Used to state that even if something else has occurred, if A happens, it negates the benefits/use of the whatever happened.
           それまで     
If I fail the next exam it's over!

Should this be modified to include たら in addition to the ば conditional form? This can be seen in some of the examples?

It certainly doesn’t help when the only official example doesn’t fit the pattern. I would replace ば with a swung dash (~), indicating a suffix. It’s a suffix that can only go after a certain kind of stem, but that can be laid out in the explanation.
